Project plan review guide
Below is a list of definitions for information that is needed to include in your request for plan review:
Project Location Details
Full Project Address: The full site address including street, city, state and country (when outside the US). If a full address is not available, provide the Lat/Long.
Index-Rec Number (should be in a format XXXXX.XX-XX):
If known, provide the FM assigned Index-Rec Number. This number can be found on any Plan Review or report completed by FM for this client specific location.
FM insured client company name: The name of the company insured by FM who has requested that FM review plans associated with this project.
FM insured client contact name/email: The name and email of the FM client company who is responsible for the project at this site.
Project Details
Project Name: The name assigned to this project that should be referenced in discussion with FM insured client contacts or contractors.
FM Project ID from previous Plan Review Letter or Project Report (if applicable): The Project ID assigned by FM on previously completed letters or reports for tracking purposes.
Brief Project Description: Provide a description of what will be addressed as part of the overall project, if the project was triggered due to a FM recommendation, expected duration, etc.
Submittal Details
Subject/Scope of Review: What is the subject or scope of the plans being submitted for review.
Example: Reroof of existing building X
Plan/Review Type (see checklist for options): Indicate what type of review is being requested; construction, automatic sprinklers, fire pump, etc.
Documents Included with Submittal: List the documents included in this review.
If resubmitting based on a previous review, Review Number from previous Plan Review Letter: If you are responding with updates to a previous plan review, please include the PR# from that letter.
Submitter Contact Information: If contact info. is not provided in your email signature, please provide your name, role, company, and phone number.
